Thank you for choosing Tracey Young and Catalyst Enterprises BC for your counselling, coaching, and consulting needs. We are committed to offering you the best care and services possible. To acquaint you with our policies and procedures, we provide the following information:
Terms of Service: Prior to engaging in professional services with Tracey Young and Catalyst Enterprises BC you will be required to read the Terms and Conditions of Service (TOS) that can be found on my website under the page “Terms of Service.” Or, read a copy that Tracey provides to you at our first in-person session.
Participating in telephone, or online services with Tracey is as if you are participating in services with Tracey in her office in British Columbia, Canada.
Appointments: Brief, limited service appointments can be scheduled for 30 minutes. However most appointments will generally be scheduled for 60 minutes. Sessions may be provided in person, by telephone, Skype, or by email.
Cancelled Appointments: We require 24 hours notice for cancelled appointments. We will require full payment for missed, or cancellation of scheduled appointments of less than 24 hours. Messages, or texts to 604-716-4102, or e-mails to firstname.lastname@example.org are valid ways of advising of cancellation.
Fee for Service: We provide a sliding scale fee for our services, which is to be paid at the beginning of each session. A receipt will be provided at the time of payment.
Methods of Payment: Cash, Interac e-transfer, PayPal, or cheque for consulting services and if special arrangements are arranged.
Third-party insurance: Generally the client will pay the service provider directly and then seek reimbursement from their insurance agency. Payment will be discussed prior to our first session.
Confidentiality and Privacy: All information you share with Tracey is confidential and will be kept private. If you are involved in legal matters, records Tracey keeps regarding our work together could be subpoenaed.
Limitations to Confidentiality: There are four conditions that are exceptions to confidentiality, these are:
1. You disclose that a minor child is being abused, exploited, and/or neglected. There is a legal Duty to Report these types of acts to the appropriate child welfare authorities in your jurisdiction.
2. You report that you have a plan to harm yourself.
3. You report that you have a plan to harm another person.
4. You report that you have a plan to commit a criminal act.
Prior to starting any services with Tracey you acknowledge you have read and understood the Terms of Service. You are encouraged to ask any questions you have, or discuss any concerns, arising from it, prior to engaging in any services with Tracey.
When you proceed with services, after having read the Terms of Service, you are providing informed consent to engage in services with Tracey. You can stop receiving services at any time.
By engaging in services with Tracey, there is an assumption that you are acting honestly and representing yourself accurately.
